Vista Grande Villa is seeking a passionate individual to assume our Director of Assisted Living & Memory Care and Independent Living role.
Primary responsibility will ensure the general well-being of residents through the delivery of high quality services in a manner that is consistent with Vista Grande Villa philosophy.
Establish & maintain procedures, objectives and work standards and is responsible for the management and supervision of the direct care and staff and all aspects of care related activities.
*ESSENTIAL JOB FUNCTIONS:* _*Resident Care:*_ 1.
Provides care management services to residents, ensuring the completion of pre-admission, assessment/reassessment, and the negotiated service plan in conjunction with the resident’s family, physician, the caregivers, and other necessary individuals.
Facilitates and encourages family involvement with residents.
2.
Evaluates and assists in developing and updating individual resident care plans with caregivers and other disciplines.
3.
Coordinates admission and orients new residents to the facility, ensuring satisfactory integration into the facility.
4.
Demonstrates an appreciation of the heritage, values, and wisdom of the residents and an understanding of the aging process.
5.
Identifies safety hazards, and institutes corrective action in a timely manner.
Shares in the responsibility for providing and maintaining an environment that is conductive to the safety, physical and emotional well-being of residents and staff.
6.
Records information in electronic health record to determine quality of service to residents and directs staff to implement needed change.
Holds daily stand-up conferences with caregiver staff and other disciplines.
7.
Monitors and maintains records reflecting the health and mental status of residents, and reports in a timely manner any changes in condition to his/her provider.
8.
Monitors medication regimen and resident’s response.
Works with resident’s provider to update medications as needed.
Works with the pharmacy consultant to ensure the appropriate handling, dispensing and destruction of medications.
9.
Develops programs/systems which ensure that resident rights and requests are honored.
10.
Works with Human Resources to investigate and document resident and employee incidents.
11.
Coordinates residency termination, discharge, and transfers when necessary.
12.
Observes and may assist in direct resident care, when necessary.
13.
Supports and participates in the resident-centered activity program.
14.
In coordination and based on recommendations from Infection Control nurse, is responsible for infection control duties.
15.
Coordinates with Executive Director to review all outside private duty and home health nursing care contract work.
16.
Must be committed to providing excellent customer service and to Vista Grande Villa’s Standard of Performance.
17.
Trains, supervises, motivates, and evaluates staff to ensure the provision of high quality services to residents in a manner that is consistent with the Vista Grande Villa philosophy.
18.
Orients new staff, develops in-service training and provides ongoing training.
19.
Plans and delegates work assignments assuring effective scheduling on a 24-hour basis.
20.
Promotes and facilitates interactive problem-solving with staff.
21.
Reviews daily assignments for resident caregivers.
22.
Ensures that all caregivers have a clear definition of their work responsibilities and their respective supervisor’s expectation for performance.
23.
Ensures that all caregivers understand their personal roles in the event of a fire or internal/external disaster, regularly reviewing fire and disaster procedures.
24.
Ensures that all caregivers understand, adhere to and maintain strict confidentiality of resident information.
25.
Encourages staff participation and communication in decision-making.
26.
Provides recognition for good work performance.
27.
Conducts individual feedback meetings with employees on a regular basis concerning performance standards.
28.
Monitors and documents performances of staff members; utilizes appropriate disciplinary measures as necessary; counsels and advises staff members.
_*Organizational:*_ * Serves as a liaison between the community, residents, families, and physicians.
* Ensures compliance with all federal, state, and local regulations.
* Initiates and monitors continuous quality improvement program.
* Assists in implementing the communities marketing plan.
*QUALIFICATIONS:* _Education: _Must be a graduate of approved school of nursing and possess a current nursing license to practice in the State of Michigan (LPN or RN eligible).
Must be able to think and function as a leader, utilizing critical thinking skills and process improvement initiatives.
Must have the ability to read, write, and speak English.
_Experience: _Must have supervisory nursing experience and sensitivity to the interest of, and understanding of the physical, mental, social, spiritual characteristics of aging, while maintaining an appreciation of the residents’ psychosocial well-being.
Must possess a high degree of interpersonal relations skills and be capable of relating to a variety of people and personalities.
Considerable initiative, judgment, and leadership skills are also required.
Must have the demonstrated ability to hire, direct, and manage the skill levels required.
Demonstrates the ability to function as a team member in sharing responsibilities for the administration of resident services in cooperation with all members of the team.
Patience and perseverance are virtues to be possessed.
A cheerful attitude and remaining flexible under stress are very important.

Contact requires the ability to communicate in English, both written and verbal, and a cooperative and courteous attitude despite simultaneous tasks.
_Errors: _Errors could result in serious physical effects to the residents in the community plus service delays, resident/family dissatisfaction or discomfort, damaged equipment, loss of service to the community, financial loss, and problems with employee morale.
_Confidentiality: _May be exposed to confidential information disclosure of which would be contrary to the best interest of the facility and professional ethics.
_Sensory: _Must be able to maintain a cooperative and courteous attitude despite simultaneous tasks in a fast-paced environment.
Must be able to function efficiently despite distractions and interruptions and be able to remain calm in an emergency.
_Physical Demands: _Must be able to move intermittently throughout the workday.
Must possess sight/hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirement of this position can be fully met.
Position requires the ability to perform complex tasks that require gross and fine motor skills.
Must have the ability to lift 50 pounds and assist in moving residents.
Demonstrate physical dexterity in moving, bending, stooping, and pulling.
Able to withstand walking or standing for long periods of time.
Must be in good general health and demonstrate emotional stability in order to cope with the mental and emotional stress of the position.
_Working Condition: _The position is generally 8am-5pm, however it is required to monitor, evaluate, and ensure performance and teamwork on off-shifts, therefore, weekly rounding on all shifts is required.
In addition, must be able to fill in on an emergency basis for any shift and commit to an on-call rotation.
Exposure to body fluids, infection, offensive odors, and to peculiarities in behavior and reactions to the aged, including combative residents.
Frequent interruptions from residents, care staff, fire alarms, telephone calls, etc.
Exposure to chemicals/cleaning solutions.
